Registration Packet
Everything you need to register your child for class at Mushroom Family Learning Center. Information includes age guidelines for each class, class meeting times and tuition schedule as well as all forms that need to be completed (Information, Emergency, Medical Release and School Contract forms). In order to register, please print out and fill in two copies of the packet, include two pictures of the child(ren) registering, registration fee and mail to Mushroom Family Learning Center 1240 Washington Road Pittsburgh, PA 15228.
